ADR-0034 §2.3 item 10: the setup effect runs through the shared orchestration

Context

ADR-0034 §2.3 item 10 (added by #19366) records the first-run setup broker's six capabilities. Its setupEffect bullet describes the state on 2026-10-02: "As recorded here, the handler refuses every call by name (unwired: …)", with the wiring named as neomjs/neo-agent-institution#440's. That leaf merged on 2026-10-03 (neomjs/neo-agent-institution#464), so the sentence is history. The follow-up was declared in that PR's body and confirmed in its review.

The Problem

The record now contradicts the shipped shell in three places of one bullet and one sentence of the custody paragraph:

• the handler no longer refuses every call: it runs the one consented effect through the Brain's shared orchestration;
• the refusal name unwired no longer exists in the shell or in the card;
• the card's row is no longer the operator's instruction for every refusal: a refusal in the orchestration's words stays on the status line, and only a shell that cannot run effects at all hands the operator the CLI;
• the custody paragraph does not say what the broker does after a record write the host rejected.

A reader who implements against the record would re-introduce the manual path.

The Architectural Reality

• learn/agentos/decisions/0034-electron-shell-architecture.md, §2.3 item 10: the setupEffect bullet and the Custody paragraph.
• The shipped contract (Institution dev@424fa0e, harness/setupBroker.mjs): effect = the settle pass, then performEffects for the one named effect in the recipe's order rules, then the re-evaluated run, as one serialized operation; a refusal reported by the orchestration answers {ok: false, reason, effectId} and writes nothing; a boot without a Brain root refuses no-brain-root. The broker holds the run's record path only and reads the record from disk in every operation; a bound record that is gone or unreadable refuses the operation.
• ADR 0041 §2 item 6 (an interrupted effect is settled by observation, never replayed) is the authority the custody sentence leans on.

The Fix

1. The setupEffect bullet states the wired contract: what runs, the two refusal shapes, and that no second implementation of the effects exists in main. The dated "as recorded here" sentence and the unwired name go.
2. The Custody paragraph gains one sentence: every record operation starts from the record on disk, so an effect whose acknowledgement write was rejected is found pending and settled by observation; a record the broker cannot read refuses the operation.
3. The item's amendment note names this leaf and its date.

Decision Record impact

amends ADR-0034 §2.3 item 10 (documentation of a shipped contract; no new capability, no change to the six channels' names or envelopes).

Acceptance Criteria

• AC-1 The setupEffect bullet describes the wired handler and its refusals; neither unwired nor "refuses every call" remains in the record.
• AC-2 The Custody paragraph states the read-from-disk rule and the unreadable-record refusal.
• AC-3 The item's amendment note carries this leaf and 2026-10-03.

Out of Scope

A fresh flag on setupEvaluate (neomjs/neo-agent-institution#475): its own amendment, ahead of that leaf, if it ships.
